Thank you for volunteering at D.A. Blodgett - St. John's. We appreciate your generosity and time! Please take a moment to read the following:

Confidentiality & Release of Information Notice
We are so excited and grateful you have chosen to volunteer your time here. We rely on the support of community members like you and your group to remain a safe and beautiful home for vulnerable children in our area.

To ensure the safety of your group and the children on our campus, there are a few things we ask you follow:

1. Have fun! We are so appreciative of your hard work, but we hope your time here is fun as well. While you are here, you will see many children playing, studying, and simply hanging out. We encourage you to say "hi."
2. Any volunteer or guest of your group must be over the age of 18.
3. Discussing a present or past client's name or situation outside of the process of providing professional services to the client is a violation of their right to privacy.
4. No photography of any child is allowed by volunteers.
5. Please do not enter any of the homes without the approval and accompaniment of one of our staff members.

Release of Information
I agree to having myself photographed, filmed, or quoted by D.A. Blodgett - St. John's for promotional, educational, or informational purposes for as long as the material is used. I waive the opportunity and right to inspect or approve any such information, photos, and/or video or any use to which it may be put.

I release D.A. Blodgett - St. John's, its directors, employees, agents, and those acting under its authority, from all claims and liabilities of any kind arising from the use of this information.
